FAQ's For Santa Cruz Medical Clinic Patients Enrolled In Blue Shield's HMO Plan For 2003

1. How do I notify Blue Shield of my SCMC physicians name?

Call Blue Shield Member Services at 800-334-5847 OR Complete a Personal Physician Selection Form, make a copy for yourself and mail the original to Blue Shield.

2. What if I have already selected a Physician's Medical Group doctor but want to "change back" to my doctor at SCMC?

A. Call Blue Shield Member Services at 800-334-5847.
B. Complete a Personal Physician Selection Form, make a copy for yourself and mail the original to Blue Shield. Note: If you choose to send Blue Shield a Personal Physician Selection Form, be sure and date the form. Any prior doctor selection will be superceded by the later dated form.

3. Do I have to call PMG and cancel my doctor selection?

No.

4. When will I get my ID card from Blue Shield?

You should receive your ID card in December. CHECK THE CARD(S), confirm that the doctor's name(s) are correct and if not, contact Blue Shield Member Services immediately at 800-334-5847.

5. What do I do if I have questions regarding Blue Shield HMO coverage?

Call Member Services at 800-334-5847 or check their website: www.mylifepath.com

6. Do I have to do anything else?

Only if you want to make other changes to your benefits package during this Open Enrollment Period.
