As the New Year approaches, we wanted to thank you for choosing the Santa Cruz Medical Clinic physicians as your primary caregivers. As we are sure you are aware, your HMO coverage through PERS will be changing on January 1st. The Santa Cruz Medical Clinic would like to be able to continue providing your care whether it is through the Blue Shield HMO or through PERS Care. If you have opted for coverage through the Blue Shield HMO, it is important for you to have notified Blue Shield of your desire to continue with the Santa Cruz Medical Clinic. If you have not already done so, contact Blue Shield Customer Service at 1-800-334-5847 and notify them of your primary care physician choice. Be sure to identify yourself as a PERS member when you are speaking with Blue Shield Customer Service.

In order to avoid unnecessary problems, check your insurance card carefully as soon as you receive it in the mail. The card should indicate the Santa Cruz Medical Clinic and the name of your chosen primary care physician. If the card indicates an incorrect physician or medical group name, please contact Blue Shield at 1-800-334-5847 to have the error corrected.

If you have already contacted Blue Shield and indicated your desire to remain with the Santa Cruz Medical Clinic, we thank you for your continued loyalty. If have been previously authorized to receive care from a physician outside of the Santa Cruz Medical Clinic, please be aware that you will need to obtain new authorizations for services after January 1st. Please contact your primary care physician or referring physician and request new authorizations if you will continue the care after January 1st.
